Today we’re going to talk about the ICP model and how it can benefit your facility. ICP utilizes an emerging business model that’s subject to limited competition at this time. To our knowledge, there’s no other company that practices under the same model. We partner with postacute and long term care facilities to provide on site, full time nurse practitioner coverage. Not only do we manage acute and chronic conditions on a daily basis that reduce hospital readmissions and ER transfers, but we work with individual facilities to attain their quality measures to improve their Star ratings. We do that by strategically collaborating daily with MDS. We’re also unique because we work with therapy teams, admissions coordinators, discharge planners from the hospitals, all to provide consistent and accurate diagnoses of skilled admissions and for these skilled residents in order to help the facilities achieve their maximum reimbursement under the new PDPM payment model. This is what sets us apart from other companies. Nursing homes, and you can look them up on the Internet, and you can Google under the Medicare website what star rating each facility has.

And a lot of people do that, that’s when they look for homes, they look up Star Rating.

Unfortunately, it’s based on the year previous data, but it’s based on several different things. So, like, their staffing needs, their infection control policy, their cleanliness, lots of different things, things they’ve gotten. So one way to improve that star status is to improve the quality of the care that the residents get. So some of the measures that are reported to CMS that kind of come up with the star rating status or things like falls, things like pressure, ulcers, polypharmacy, what are some others? Hospital readmission, hospital remission..

Returning to the hospital, to the ER for not really necessary reasons kind of thing.

All of those things, our nurse practitioners are in place in a facility, can reduce all of that across the board. So over time, that star status increases. And not only does that increase their reputation in the community, where more people want to send their residents there, it increases the profit that that facility is able to bring in.

Yeah. ICP’s claim to fame is the fact that our nurse practitioners are there for the home. They’re there representing the home to improve quality measures. That’s a big reason that a lot of homes have us come in and help them is to improve their quality measures. And we really focus on that as providers.
