Sterlington’s newest primary care clinic is now open to the public.

Integrated Care Professionals’ Clinic at Gardens of Somerset is Sterlington, Louisiana’s only Nurse Practitioner clinic specializing in geriatric care. We provide primary medical care to adult residents of the community as well as residents and staff of Gardens of Somerset.

Participants will complete a detailed cognitive assessment with the Nurse Practitioner, using an evidence-based tool called the BCAT. Use of this tool provides a multidimensional understanding of resident needs and strengths. The information provided from the BCAT allows the Physical Therapy Team to tailor training, exercises, and other forms of therapy to the patient’s needs. It also provides specific treatment recommendations for facility staff as well as other healthcare providers.

Reflection Program

Specialized memory and cognitive assessments coupled with physical therapy to detect early signs of cognitive decline and protect against preventable functioning and memory loss.

The clinic at Gardens of Somerset is now open to the public for primary medical care and preventative care. Our full-time nurse practitioner, Elizabeth, is here to assist in everything from minor illnesses and injuries to routine check-ups, vaccinations, and more.

Meet Our Nurse Practitioner

Elizabeth Hoskins, APRN, FNP-C is a Family Nurse Practitioner.  She received her Associate of Science in Nursing from Louisiana Tech University.  She then later received her Bachelor of Science in Nursing and Masters of Science in Nursing from the University of Louisiana at Lafayette.  She is board certified as a Family Nurse Practitioner through the American Association of Nurse Practitioners.  She has 20 years of overall nursing experience, including emergency room, cardiac catheterization laboratory, family practice, urgent care, pediatrics, and medical aesthetics.  She is a member of the American Association of Nurse Practitioners.

Elizabeth is a lifelong native of Northeast Louisiana, and resides in Monroe with her husband, Jason.  She has a 9-year old daughter, with whom she enjoys antique doll hunting.  Elizabeth also enjoys traveling, cooking, gardening, and spending time with her family.  She is a member of St. Paul’s United Methodist Church.
